Volunteer/Intern/Mentor Liability Waiver
I understand that I am offering my services to the Genesis Hopeful Haven without compensation.
I specifically assume all risks of injury arising out of my presence on or about the premises, or my use or intended use of the equipment and facilities, or my participation in the activities of the Genesis Hopeful Haven and do hereby for myself and my heirs, executors and administrators waive, release and agree to hold free from all claims for damages Genesis Hopeful Haven and its respective officers, directors, board of members, trustees, members, employees or agents. I understand that Genesis Hopeful Haven is not responsible for personal property lost or stolen while volunteering or working as an intern on Genesis Hopeful Haven premises.
I give my permission to Genesis Hopeful Haven to use limitation and obligation, photographs,
film footage, or tape recordings which may include my image or voice for purposes of
promoting or interpreting Genesis Hopeful Haven programs, events, and/or activities. I agree this waiver includes releasing Genesis Hopeful Haven for all actions, specifically including claims based on the negligence of the Genesis Hopeful Haven or its employees and agents.
I agree to conform to the rules and regulations of the Genesis Hopeful Haven and the State of
Florida and acknowledge that these rules and regulations may be changed at any time, at
Genesis Hopeful Haven’s sole option and without prior notice.
Additionally, as being approved as a Genesis Hopeful Haven volunteer/ intern/mentor, you maybe randomly selected to submit to the digital fingerprinting process as conducted by Genesis Hopeful Haven.
By signing this volunteer/ intern/mentor liability waiver, I,
understand and agree to the above provisions.
