I understand that I am responsible for informing the nail technician of any medical conditions or allergies that may affect the services provided.

I hereby grant permission for pictures and videos of my procedure to be taken. I understand and agree that these images may be used for record-keeping purposes and/or promotional and advertising materials.

During the treatment, despite all the precautionary measures made by the artist, injury is possible. I will not hold Yagudayeva, Milla's Hair Salon, or its affiliates who performing this service on me responsible for any issues that may arise because of having the procedure performed on me.

I understand that the nail technician is not responsible for any complications or damages that may occur as a result of my failure to follow aftercare instructions or to disclose relevant medical information.

I understand that I am responsible for following any aftercare instructions provided by the nail technician.

I hereby consent to the nail services described above.
