For good and valuable consideration, receipt of which is hereby acknowledged, I authorize Haven Innovation, Inc. (“Producer”) and its respective parents, affiliates, subsidiaries, licensees, successors, and assigns to make use of my appearance in social media for use by and in connection with Producer or otherwise.

I agree that you may tape and photograph me, and record my voice, conversation and sounds, during and in connection with my appearance and that you shall be the exclusive owner of the results and proceeds of such taping, photography, and recording with the irrevocable right, but not the obligation, to use, copyright and license others to use, in any manner and in any and all media, an unlimited number of times in perpetuity, throughout the world, all or any portion or reproduction thereof, in connection with the Project or otherwise, including, without limitation, use in the promotion, advertising, sale, publicizing, and exploitation of the Project and/or otherwise and ancillary products (e.g., merchandise).  For purposes of clarity, I expressly waive any and all moral rights I may have in connection with my appearance.  I further represent that any statements made by me during my appearance are true, to the best of my knowledge, and that neither they nor my appearance will violate or infringe upon the rights of any third party.

I understand that Producer is not a signatory to any union or guild or other collective bargaining agreements that may be construed to relate to my appearance and acknowledge that my appearance in the Project does not fall under the jurisdiction of any SAG or AFTRA agreements (whether or not I may be a member of either such guild).  My appearance and participation in any aspect of the Project is not a performance, and I am not portraying any role or part or taking direction as a performer, but rather appearing as myself.  My appearance in the Project is as a non-performer only, and is not employment, and does not entitle me to wages, salary, or other compensation under any collective bargaining agreement or otherwise.

This Agreement constitutes my entire agreement, and the only agreement, with Producer concerning the Project, and replaces any previous oral or written communications or understandings we may have had.  This Agreement can only be modified by a written document signed by me and Producer.  This Agreement and any variation or modification hereof shall be governed by and interpreted in accordance with the laws of the State of Michigan.  I have read this Agreement and am fully aware of its contents.  This Agreement shall be binding on me and my legal representatives, heirs, successors, and assigns

FOR MINORS ONLY:

I certify that I am the parent or legal guardian of the Minor, and I have the legal authority to execute this Appearance Release and grant the rights stated above on behalf of myself and the Minor. This Release is binding on me, the Minor, and our respective legal representatives, heirs, successors, and assigns.
