BASIC GROOMING PACKAGE. Included Services: Oatmeal shampoo and conditioning, brush/blow out, nail trimming and filing, ear cleaning. Prices vary by weight. Additional charges may apply for matting, temperament etc.

WAIVER & CONSENT

I certify that I am the owner of this pet.

I assure Anchor's Ark Pet Resort that the health information for this pet listed within the document is accurate and true, and no vital health information has been omitted.

I confirm that my pet has a complete and updated vaccination.

I confirm that my pet doesn't have any flea or any communicable disease.

I release Anchor's Ark Pet Resort, its employees, and owners from any liabilities including, but not limited to, injury, sickness, damage, accident, or death while in the vicinity.

- If I'm unable to pick up my pet on time, I will notify Anchor's Ark Pet Resort.

- Additional charges may apply.

- I understand that my reservation is not confirmed until I receive an official confirmation email from Anchor's Ark Pet Resort.

- I grant permission to Anchor's Ark Pet Resort and its staff to obtain on my behalf and in my pets' best interest the veterinary care necessary to treat illness or injury.

- I agree to pay all veterinary and other necessary services incurred by and for my pet during its stay at Anchor's Ark Pet Resort.

- I agree to pay all charges the day I pick up my pet and I understand that my pet may not leave the premises until all charges are paid in full.

- I understand that any animal left for 48 hours, beyond the estimated date of pick up, will be considered abandoned. Anchor's Ark Pet Resort will then gain ownership of the pet with full rights and responsibilities, to make decisions that are in the best interst of the pet.
