BY CHECKING THIS BOX I AGREE TO THE TERMS BELOW:

For good and valuable consideration, the receipt of which is hereby acknowledged, I hereby grant Dirty Dough, LLC and the Deeper Than Dough podcast (collectively, the “Podcast”) permission to use my likeness, image, voice, and/or appearance as such may be embodied in any photos, video recordings, audiotapes, digital images, podcasts, social media posts, and the like, taken by or made on behalf of the Podcast or its employees, contractors sponsors, partners and/or representatives. I hereby irrevocably authorize the Podcast to edit, alter, copy, exhibit, publish, sell, prepare derivative works, advertise, publicize and/or distribute any photograph, audio, recording, and/or video using my likeness, image, or voice for purposes of publicizing the Podcast or for any other related lawful purpose. These include, but are not limited to, the Podcast’s website, mobile applications, social media, marketing materials, printed and digital newsletters videos, advertisements, news releases, and any promotional materials in any medium. All edits, productions, and publications are under the sole discretion of the Podcast. The Podcast shall also own the worldwide rights, title, and interest, including copyright in, and to, these audio and video recordings.   In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I acknowledge that I will not receive any compensation for the use of such images, video, likeness, etc. and waive any right to royalties or other compensation arising or related to the use of the photographs and/or videos.    I hereby hold harmless and release and forever discharge DIRTY DOUGH LLC, The Podcast, its CONTRACTORS, EMPLOYEES, agents, and representatives FROM any and all claims and demands arising out of or in connection with the use of my likeness, image, voice and/or appearance, including any and all claims for invasion of privacy, right of publicity, misappropriation or misuse of image, and/or defamation. I represent that I am over the age of eighteen (18) years and that I have read the foregoing and fully understand its contents. This release shall be binding upon me, my heirs, legal representatives, and assigns. This agreement is being made and entered into under the laws of the State of Utah and shall be governed and interpreted in accordance with the laws of said state.
